Do I have to pay the balance on my bill from the doctor?
You should have received an “Explanation of Benefits” (EOB) in the mail that breaks down what was covered by the plan and what you are responsible for paying, if anything at all.

What are my copays for medical visits?
Plan A

  • PCP office visit: $20
  • Specialist: $30

Plan B

  • PCP office visit: $20 + 10% coinsurance
  • Specialist: $30 + 10% coinsurance

Is a pre-certification required for an inpatient admission?
Yes. Your provider must contact Blue Cross Blue Shield of MA for a medical admission and HMC Healthworks for a behavioral health or substance use admission.

How do I request a new card (medical, dental, or prescription drug)?

  • Dental card: Contact Delta Dental at 1-800-872-0500
  • Prescription drug card: Contact Express Scripts at 1-800-467-2006
  • Medical card: Contact MLBF at 1-800-342-3792 or 781-272-1000

Will my coverage continue once I stop working?
If you no longer have enough hours in a qualifying period to maintain eligibility, you may elect COBRA continuation coverage.
