Partnering with Conduent on Medical Claims
If you had a recent doctor’s visit, you may have received a letter from Conduent asking you to fill out a survey related to a recent medical appointment. Don’t worry – it’s not a scam! MLBF is working with Conduent to validate medical claims where a third party, like an employer or an auto insurance company, might have to pay your claim.
Here’s what you need to do. When you receive the letter, go to https://TPLResponse.com and enter the file number associated with your claim. This will bring up a survey with questions about your claim. Please fill out and submit the survey to ensure we have the most accurate information about the medical treatment you received and its causes.
